I acknowledge that participation in activities at this facility (Provo Indoor Soccer) involves inherent risks, including but not limited to physical injury, illness, or property damage. I voluntarily assume all such risks on behalf of myself and any minor participant for whom I am the parent or legal guardian.
I hereby release, waive, and discharge the facility, its owners, operators, employees, contractors, and affiliates from any and all claims, demands, or causes of action arising from participation in activities at the facility, whether caused by negligence or otherwise, to the fullest extent permitted by law.
I certify that I am physically capable of participating (or allowing my minor child to participate) and agree to abide by all facility rules and policies. I understand that participation is prohibited without a completed waiver on file.
